FAHSI College Scholarship Program (Application Form)

Dear Applicant,

Established in 1993, the Filipino American Human Services, Inc. (FAHSI) is a nonprofit, community-based organization dedicated to serving the Filipino Community of New York City. This year, FAHSI is pleased to offer the FAHSI College Scholarship Program (FCSP). The program’s goal is to encourage Filipino youth to pursue higher education and have a positive impact in the Filipino Community.

FAHSI is offering a $5,000 scholarship to one Filipino/Filipino-American graduating high school senior who demonstrates a passion for business and service to the Filipino American community. In addition, during the student’s matriculation at a college, the recipient will have the opportunity to receive mentoring from Frederick and Demi Seguritan, FAHSI Staff and members of the FAHSI Board of Directors.

To be eligible to apply, you must be able to demonstrate that you: · Are of Filipino Heritage (please inquire about definition); · Reside in the Tri-State Area; · Are in good current academic standing at your high school; · Have applied for admission to a four-year college as a full-time student; · Have a passion for business (you are not required to major in business in college);· Are active in the Filipino Community (i.e. participation in FAHSI’s youth program or a high school’s Filipino student club); and · Can demonstrate financial need.

To apply, please complete the attached FCSP Application and provide supporting materials, which include an essay, two letters of recommendation, a resume, a copy of your official high school transcript, Free Application for Federal Student Aid (FAFSA), and SAT scores. FAHSI must receive your application by Sunday, June 15, 2008. Please mail the application package to: FAHSI, Philippine-American Center, 185-14 Hillside, Avenue, Jamaica, NY 11432. No faxes please.
